Medical ID Card

Motorcycle Medical ID™ card has ALL the information you really need, even for diabetics with either Type 1 or Type 2 diabetes!

A note about Blood Type – the field for blood type was omitted from the emergency Motorcycle Medical ID card because it is no longer important.  Blood type is now easily, quickly and routinely obtained in the emergency room. You won’t even be asked because blood typing in the emergency room is more accurate since people often make mistakes (see What’s My Type? by John A. Vaughn, MD, Student Health Services, The Ohio State University http://shc.osu.edu/blog/whats-my-type).

How to save on your computer with your information – Motorcycle Medical ID™ card is an easy to use PDF file which you save on your computer.  Adobe Reader will not let you save your personal information.  However, you can save your personal information (on your computer) if you download Foxit, the free Adobe Reader alternative which gives you more features than Adobe Reader and we believe it’s less vulnerable to viruses.

How to use:

  1. Download your Motorcycle Medical ID™ card.
  2. On the download page, click on the download link (not the image).
  3. Firefox: click on Save File; IE: click on Save; Chrome: right click on the PDF image and click on Save As.
  4. Save in a folder on your computer where you can find it again to print a new one when it gets worn.
  5. Fill in your personal and medical information.  Using Foxit free PDF reader, just save the file and your information will be saved and can be changed for next use.
  6. HOW TO PRINT your Motorcycle Medical ID card in WALLET SIZE:
    • If using Foxit,
      • in the Print screen, under Print Handling >Scaling Type:
        • Select “None” in the drop down box and
        • Make sure Auto-Rotate and Auto-Center is checked.
    • If using Adobe,
      • in the Print screen, under Page Handling >Page Scaling:
        • Select “None” in the drop down box and
        • Make sure Auto-Rotate and Center is checked.
  7. Cut around the edge of the card and fold in half.
  8. Optionally laminate your Motorcycle Medical ID card.

If you want to laminate your card, it’s best to bring it to a store like Staples (Staples locations), OfficeDepot (OfficeDepot locations), or UPS (UPS locations) where they can do a professional job.

You can also purchase self-adhesive laminating sheets. Place between two laminating sheets and then cut slightly larger than the Motorcycle Medical ID card so that the laminating sheets contact each other around all four edges of the card.

You can purchase self-adhesive laminating sheets at a store like Staples or on the Internet:

Avery Self-Adhesive Laminating Sheets 3 mm thick, 9 x 12 Inches – 2 Pack 73602

Avery Self-Adhesive Laminating Sheets 3 mm thick, 9 x 12 Inches – 10 Pack 73603 (Recommended)

Avery Self-Adhesive Laminating Sheets 3 mm thick, 9 x 12 Inches – 50 Pack 73601

Ride a bicycle?  The Motorcycle Medical ID card is also great for bicyclists and anyone on the road.

Make sure you protect yourself with uninsured/underinsured coverage on your motorcycle insurance policy which will pay you when you’re injured in a motorcycle accident by someone who doesn’t have insurance or doesn’t have enough insurance. We recommend uninsured/underinsured limits of $250,000, $300,000, $500,000, or more. To see how much you need, use this uninsured/underinsured coverage calculator.
